Currently we have Version 9 installed and wish to upgrade to Version 12.
Are there any best practices to upgrade from Version 9 to Version 12?
Has anyone encountered issues upgrading from version 9 to version 12?

Generally you have to go though all version step-by-step
v9 > v10 / v10 > v11 / v11 > v12
if you search for upgrade from v10 > v11 using the forum search I am sure you will find the steps required. I am not aware though whether you can just adjust those v10>11 steps to each step in your v9 > 10 > 11 > 12 ladder or whether the methods between upgrade steps may vary.
I guess looking for individual instructions for each step may be the best option.
Needless to say that you can not do this directly on your production system but need to follow a thorough backup/snapshot routine for each step in order to not run into a disastrous situation where you get stuck forward and have nothing to step back to.
