Cannot access new-stock-entry Doc after customization

I customized the stock entry form and modified Qty field to support 5 decimal place precision but after that I am not able to access it.

It shows the following:

What to do and how to get this to function properly?

I managed to solve this.

I went to Customize Form and selected relevant form and opened it for customization and then I clicked on Actions->Reset to defaults and then clicked on Update to save changes.

Now the form is working!
