Upload Files via Document in a Specified Folder

Hi Everyone,
I have few requirement which are:

  1. I want to upload list of files under a specific folder so that it becomes easy to access those files which are related to a particular product/project.
  2. Also, it would be more helpful, if a folder get created when I create a new task/project.
  3. Also, it would add an advantage, if I get an option to specify the folder name with the press of a button.
