System seems to upload attachments twice automatically

Dear Community,

I am unsure what went wrong.

I attached a file. ERPNext/Frappe it seems uploaded the file twice.

Can anyone tell me what went wrong?

Some issue is there. To replicate the issue, I attached a file to a document. Error message said.
