When I try to create my doctype, i can’t find my input for fill information. What should be done for fix it?

Have you reloaded the form?
It is always good to reload the form once you’ve updated the doctype.
Also, if you’ve added a new field in a custom doctype, you should add the same via the doctype and not customize form.
I already done it, isn’t work. It happened not first time and i tried to figure out, why these fields are dissapear. Anyway, thanks for your activity)
