Multiple form field with same name: **customer_address**

I have multiple form field with same name: customer_address in Purchase Order form.

Kindly help to remove one field. As it is not getting deleted.

one of the field is standard field. and another one was created by me few days back.

Kindly help.

@Ram_Gopal_Rao just delete your custom field. What is the error?
