We should be able to replicate it at our end. So please give us as much information as you can. Please see it from the point of view of the person receiving the communication.
For sending images, use http://imgur.com or other similar services. Do not send images as attachments. Links are good. Same goes for any file you are going to send.

Thanks for the update, another question i have regarding prices and customers is, is it possible to retrieve prices for a particular item and particular customer during invoicing, sales order, quotation, proforma?
That would be really helpful, as we have repeat business from customers and all may not have list price as their quoted price. Any tips/solutions?

Isn't the item form already big? You can get a much more organised data using a report. I think one already exists. You can use it to filter by item or price list.
