How to remove item name from material request

Hello ,
When i create material request ,1st select the type then select the item after saving the it can show whole name bases in type selected like this

purchase request for item test
i dont this i want to remove the item name from here

for same as all type
can any one help me for this

You can customize the form and set the Title Field to the field you want to show as title, or leave bank to use the name field of the document

actually is is based on the type which we selected , in the material request there are 5 type based on that type it can save the name suppose i select the type purchase then it can be save like this
purchase request test-item so i want only selected type not include the item name …

There will me multiple number of same request type. How do you plan to change it?

What do you see in customise form?

if i select type material transfer then after saving it can show only the
material transfer request like this

purchase request for test item instead of that i want to show only the type name like this
for purchase type it can show only purchase request
for material transfer it can show materail transfer request
