Create Custom Report for Items lost in transition

Hi, I need help with a feature of ErpNext and am not able to figure out the process.

For a manufacturing company, suppose some user who is in charge of production makes 10 units of an item. He updates the stocks and then some other person who is delivering the item updates that he has delivered only 9 items. 1 item is either damaged in delivery or he has stolen one item. So how do I incorporate this into my erpNext system?

I also want to make a chart or any graphical representation comparing units of the item going out of warehouse and units of the item actually delivered to the customer?
